Default Data Center redesign

I am undertaking a project of streamlining our data center to make it more structured. Currently it seems there was an attempt at structured cabling, however it fell short and is now un-maintained to the extent of confusion. When a server is installed or decommissioned it's always a treasure hunt to track down cables to see where they should connect or disconnect. I am now documenting all cables and cable numbers to find and document their far end connections. However, I would like to redesign the Cabling system in place to bring it to a more standard structured cabling system.
My problem is lack of experience, what I know is all from self study as this is my first job experience managing a data center. How would I begin? What factors should I look at, what kind of trouble will I find myself in during this approach?
I'm seeking advise from the more seasons pros out there.

Maybe it offers rather viable pieces of advice, but I don't think that it worth purchasing in such a case. There is a lot of up-to-date information on the Web - so why not to search for the one?
