Generally, in any of the service providers I've worked in, we would have high level filters to all servers for customers allowing only the ports they request, or full access if they actual request this.
For our own services, we lock down with a double skin in most instances with dual vendor at the perimeter - should there be a vendor specific exploit we still have our safeguards in place.
This all obviously comes at a price (generally filtering bearing negligible, double skins $$$) although I am slightly sceptical of articles of this nature that potentially act to over exaggerate a position.
