Datacentre Profile Document

I am trying to create a document which will contain the following information:

- Site Address
- Site Location
- Access Procedures
- Cabinet Layout Diagram
- Equipment Inventory
- Power Inventory
- Patch Panel Inventory
- Circuit Inventory

Does anybody have a nice template for a document like this?
